Charge LPN - Brookside Memory Care ResidenceCross Keys Village - The Brethren Home Community (CKV) is currently seeking Licensed Practical Nurses to become an integral leader on our Brookside team and help provide our residents with the highest quality of care.

Our current needs include:
Day Shift Weekend Option working 6:
00 am - 2:
30 pm every Friday, Saturday and Sunday.
(Benefits Eligible Position) Part-time Day Shift; working 6:
30 am - 2:
30 pm Weekend and holiday rotation required.
PRN; picking up shifts as needed.
Weekend and holiday rotation is required.

Our Brookside Memory Care Residence is a specialized personal care environment for individuals with a diagnosis of neurocognitive disorder or dementia, designed to provide an optimal environment during the intermediate stages of their disease.
CKV team members work together to provide Brookside residents with guidance, cueing, the right level of stimulation and interaction with others who understand the progression of their condition.
Job Description:
As a Charge LPN, your scope of responsibilities will include but is not limited to:
Identifying, coordinating, implementing, evaluating and monitoring health services for Brookside residents.
Providing guidance and leadership to Brookside team members.
Participating in the resident admission process to ensure tasks are completed timely and appropriate support plans are implemented.
Identifying and performing in-depth assessments of resident's physical status and referring residents to a physician or appropriate agency as medically appropriate.
Completing required documentation and record keeping accurately and timely in an electronic care tracker.
Assisting physicians on resident appointments, following physician orders and ordering medications as prescribed.
Education and
Experience:
Must be licensed as a practical nurse in the State of Pennsylvania.
Post-licensure experience preferred but not required.
Must demonstrate CPR competencies.
Evidence of continuing education in line with community, state and federal regulations.
Must possess quality mindedness, critical thinking, initiative in leadership responsibilities, interpersonal sensitivity and communication skills.
